Below is a list of all application materials that may be required by the program to which you are applying.

  • Application form (print out) or DOCX File
  • Statement of purpose and area(s) of interest (1 page) 
  • Three recent 2x2 inch photograph ( white Background)Jpeg or PNG File
  • A copy of transcript record for your Bachelor’s degree and/or any higher degree(s) obtained (certificate/transcript/grade-sheet) from your previous university.
  • Report of English test score (one or the other)

    8.1.A: Graduates from universities where English is the language of instruction
     or from international programse accredited by OCSC Thailand are eligible for waiver. (Please self-check and print the page for certification).

    8.2 Waiver for Scholarship Recipients
    Scholarship recipients may be eligible for waiver subject to approval from the College of Public Health Sciences Committee.However, enrollment in the course must meet criteria 4-5 as outlined in the Standard English Admission Criteria, 2014 (October 28, 2014).

    1. Applicants who come from a country where English mother tongue language (USA, UK, Canada, Australia, New Zealand)are exempted from both admission & exit examination English test. 
    2. Applicants who got a scholarship from any funding Agency are exempted from admission English test but still need to submit exit examination
    3. Applicants who have a previous degree taught in English approve by OCSC are exempted from both admission & exit examination English test. **
    4. Applicants who hold a previous degree taught in English, accompanied by a certified letter from their previous university, may be exempted from the admission examination. They may alternatively opt for either the exit examination or an English proficiency test, with the final decisionresting with the CPHS committee's discretion
    5. Students who choose to waive the English admission criteria as described above will not be eligible to request tuition fee scholarships. However, it's important to know that CPHS still provides various scholarship options during studies, including research funds and international presentation funds.
    6. Students who opt to fulfill the English admission condition by enrolling in an English course must be aware that they need to achieve a minimum score on an English test for comparison. Additionally, it is mandatory to take the examination once to determine their minimum standard.

    4.2.B) Master Degree Program students with a Score
    -CUTEP 30 ≥ 38
    -TOEFL 400 ≥ 425
    -IELTS 3.0 ≥ 3.5

    These students must enroll in and successfully complete 2 courses by listed below to meet the English exit exam requirement before requesting the thesis examination:

    1.5500503 Preparatory English for graduate (mandatory)
    & Choose one course from the following list:

    • 5500504 English Pronunciation and Conversation
    • 5500505 Academic English Grammar
    • 5500506 Academic English Vocabulary
    • 5500510 Skills in English for Graduates

    4.2.C) Master Degree Program students with a Score
    -CUTEP 38 ≥ 45
    -TOEFL 425 ≥ 450
    -IELTS 3.5 ≥ 4.0
    These students must enroll in and successfully complete one course from the following list to meet the English exit exam requirement before requesting the thesis examination:

    • 5500504 English Pronunciation and Conversation
    • 5500505 Academic English Grammar
    • 5500506 Academic English Vocabulary
    • 5500510 Skills in English for Graduates.

    5.2.B) Doctoral Degree program students with a Score
    -CUTEP 45≥ 60
    -TOEFL 450≥ 500
    -IELTS 4.0≥ 5.0
    These students must enroll in and successfully complete 5500532 Academic English for Gradute Study &  5500560 Thesis Writing to meet the English exit exam requirement before requesting the thesis examination


    5.2.C) Doctoral Degree program students with a Score
    -CUTEP 60≥ 67
    -TOEFL 500≥ 525
    -IELTS 5.0≥ 5.5
    These students must enroll in and successfully complete 5500560 Thesis Writing to meet the English exit exam requirement before requesting the thesis examination.

    **Due to the specialized enrollment process spanning across faculties, students are required to submit a written request to the Academic Section of CPHS at least one month before the commencement of the semester. This allows us to reserve seats accordingly 
    Students who do not meet the English proficiency standard are ineligible to proceed to their thesis examination. It is incumbent upon them to regularly report their progress in improving their English scores to their advisor during research project consultations. Any oversight resulting from the lack of English proficiency ultimately falls on the student, as CPHS offers various protocols for assessment, which students are expected to adhere to diligently Please reiterate, as this message is of utmost importance, submission of English proficiency scores is required prior to thesis examination
